Providing Primary Care At Scale
The London Medical Associates (LMA) aims to support local practices by providing primary care services at scale across West London. These services provide additional capacity to ensure increased availability of appointments for our residents or offer specialist services to our patients that could not be offered at a practice level. Our teams partner with local GPs, practice staff and other NHS and social care staff to ensure our appointments are accessible, expertly delivered and sensitively delivered for our residents.
Urgent Integrated Care:
Clinical Lead: Dr Amit Patel
The London Medical Associates’ Urgent Integrated Care service provides a bespoke home visiting service for our local frail and elderly residents. Through expert clinical support and decision-making, our team works in partnership with our rapid response, palliative care, care homes, and local practices to ensure these patients receive the highest quality care, reducing the number of unnecessary ED attendances and hospital admissions within our local hospitals. We ensure that patients can be treated within the safety and comfort of their own homes whenever possible
Extended Hours GP Access HUB:
Clinical Lead: Dr Meera Pai
Our London Medical Associates (LMA) Extended Hours HUB is the beating heart of our Federation’s Primary Care at Scale Division. We provide out-of-hours and emergency primary care appointments for our 40 Federation practices and 111/999 services across the Royal Borough of Kensington & Chelsea and North Westminster. Operating 7-days a week into the evening and throughout weekends & bank holidays, the service provides patients access to GP and nursing provision when they need it the most.
Operating out of St Charles Integrated Care Centre and Violet Melchett Centre, the service provides equity of patient access across West London
RM Partners: Cervical Screening Project:
Clinical Lead: Dr Meera Pai
Working in collaboration with RM Partners, the London Medical Associates (LMA) Cervical Screening project aims to increase the availability of cervical screening appointments for our residents. The LMA has successfully supported local practices to increase uptake by providing innovative booking methods and appointment provision in off-peak hours. It allows patients not only to be seen at a time that suits them but also to book at a time that suits them.
Grenfell Community Enhanced Health Checks:
Clinical Lead: Dr Thushara Goonewardene
London Medical Associates (LMA), in partnership with NWLCCG, have been commissioned to continue the crucial healthcare provision for bereaved, survivors and the wider community affected by the Grenfell tragedy Enhanced Health Check clinics. These clinics ensure that the broad spectrum of complex health needs presented by this group is not only met but proactively managed in a timely and culturally sensitive manner. The Federation has set up multiple self-referral approaches for residents to access these clinics to streamline the access route for patients and encourage a proactive healthcare ethos within the Grenfell community.
Community Respiratory Hublet:
Clinical Lead: Dr Meera Pai
Commencing in February 2022, London Medical Associates (LMA) will increase the provision of Community Respiratory appointments within West London. The Respiratory Hublet offers local service for our 40 practices and their patients. Working alongside our PCN colleagues and Imperial’s Community Respiratory nursing team, the LMA aims to promote ARTP training across the borough. Enabling local movement will mean rapidly creating local capacity to pre-pandemic levels.
Out of Hospital Services:
Clinical Lead: Dr Meera Pai
The range of Out of Hospital Services that the Federation provides to our practices are intended to:
Increase routine service capacity, i.e. cervical screening
Enable speciality services at scale, i.e. spirometry
Support localised provision of enhanced services, i.e. ECGs & point of contact (POC) testing
Organise engaging and educational events to ensure that local GPs have the updated knowledge that they need to provide enhanced patient care
