Diabetes Workshop comes to West London

The Diabetes 2 Model workshop took place on the 13th February and was well represented by a range of healthcare professionals not limited to GP’s, practice managers and nurses from across the Primary Care Networks (PCN’s).

In addition to this there was also a strong representation from West London CCG, Central London Community Healthcare (CLCH) and North West London (NWL) CCG and Imperial College Health Partnership (ICHP) the workshop facilitators.

The feedback from the workshop has been positive and in the next couple of weeks, the workshop outputs will be shared giving clarity around outcomes achieved.

This is a great example of how collaborative working, PCNs and Federations can drive better healthcare for local patients.
